Among those who worked in the same building was Harvey Lillard, a janitor whose cheerful demeanour masked a profound struggle. For seventeen years, Harvey had lived in a world of muffled silence. His hearing loss had begun suddenly during what he described as an awkward bending motion while working, and despite seeking help from various practitioners, his condition remained unchanged.

On September 18, 1895, the paths of these two men intersected in a moment that would echo through medical history. As Harvey went about his janitorial duties, D.D. Palmer engaged him in conversation, learning about the circumstances surrounding his hearing loss. Harvey explained how his deafness had begun abruptly after feeling something “give way” in his back while stooping over.

Palmer’s trained hands explored Harvey’s spine with methodical precision. What he discovered fascinated him—a vertebra in the upper neck region that seemed displaced from its natural position. “I reasoned that if that vertebra was replaced, the man’s hearing should be restored,” Palmer would later write in his journals.

The Posture Check: What Your Mirror Knows

Your posture tells a story about what’s happening with your spine—and it’s a story you can read yourself with nothing more than a mirror and a moment of honesty.

Try this quick posture check:

  1. Stand sideways next to a mirror (or have someone take a side photo of you)
  2. Look at your natural standing position
  3. Notice the position of your ears in relation to your shoulders

In ideal alignment, your ear should line up directly over your shoulder. For every 2.5 cm (1 inch) your head moves forward from this position, the effective weight on your spine increases by about 4.5 kg (10 pounds). That’s like strapping a bowling ball to your neck!

If you notice your head jutting forward (what we call “forward head posture”), your neck is working overtime, and the joints in your cervical spine are under abnormal stress. This isn’t just about looking slumped—it’s about spinal joints that are stuck in unhealthy positions, unable to move through their normal range.

The Range of Motion Test: Your Neck’s Cry for Help

Your neck should move freely in multiple directions. Limited range of motion isn’t just inconvenient—it’s a sign that your spinal joints aren’t gliding properly. Here’s a simple self-assessment:

  1. Sit comfortably and try to touch your chin to your chest
  2. Look up toward the ceiling
  3. Turn to look over each shoulder
  4. Tilt your ear toward each shoulder

Ever had one of those days where you just can’t think straight? Where focusing feels like trying to catch fog with your bare hands? Before you blame it on a lack of sleep or too much stress, you might want to check something else: your posture.

The Weight of the World (On Your Neck)

Look around next time you’re in public. What do you see? People hunched over phones, necks craned forward, shoulders rounded. We call this “forward head posture,” and it’s become an epidemic in our digital age.

Here’s what’s happening: Your head weighs about 4.5 – 5.5 kg (10-12 pounds) when it’s balanced properly atop your spine. But for every 2.5 cm (1 inch) your head moves forward, the effective weight on your spine increases by about 4.5 kg (or 10 pounds)! That means if your head is just 7.5cm (or 3 inches) forward (which is common), your neck muscles are dealing with the equivalent of an 18 kg (or 40-pound) head.

 

Your Neck: The Highway to Your Brain

“But what does this have to do with my brain?” I hear you asking.

Everything, actually. Your spinal cord is like the main highway connecting your brain to the rest of your body. All those important neural signals travel up and down this pathway. When your head juts forward, it creates tension and compression in your upper cervical spine, right where it connects to your brain stem.

This compression can:

  1. Restrict blood flow to your brain (and your brain is kind of a blood hog, using about 20% of your body’s oxygen supply)
  2. Put pressure on nerves that affect cognitive function
  3. Trigger inflammation that impacts how clearly you can think
  4. Disrupt cerebrospinal fluid flow that bathes and nourishes your brain

Brain Fog Is Real

That mental cloudiness you feel after hours at your desk? It’s not all in your head—well, actually, it is, but it’s likely connected to your posture! When your posture is compromised, your brain has to work harder to process information while receiving less of the resources it needs.

Many of my patients describe it as feeling like they’re “thinking through molasses.” They struggle to find words, lose their train of thought, or find it hard to concentrate. And often, they never connect these symptoms to their forward-leaning posture.
